Excellent discussion. The AHA Distinguished Scientist lecture given this week by Dr Rob Califf promoted the value of very large controlled trials. Unfortunately, these are also very expensive. I sat next to Dr Jay Cohn (one-time Director of Cardiology at the U of MN) at the lecture and he pointed out that small trials in which the subjects are precisely categorized ("deep phenotyped")can yield valuable information at much less cost.
