History

Several needs assessment studies conducted between 1998 and 2000 in the Kingston, Frontenac, Lennox and Addington (KFL&A) region identified key care delivery problems e.g., fragmentation of palliative care delivery, across all organizations. In response to these problems, the KFL&A Palliative Care Integration Project was initiated in January 2001 to ensure efficient, high quality palliative care delivery to terminally ill cancer patients through development of service integration across the continuum of care. It is anticipated that improved continuity of care and reduced variability of palliative care services will be achieved through:

  1. the development of evidence-based collaborative care plans (CCPs),
  2. the development of evidence-based Symptom Management/Medical guidelines,
  3. the use of common, validated assessment tools, and
  4. application of the CCPs, medical guidelines and assessment tools in the different care settings in the region

Current Status

Cancer Care Ontario and the Ministry of Health and Long-Term-Care have identified palliative care as a strategic priority. Consultations were held around the Province of Ontario and it was agreed by 12 regions to implement the PCIP as a quality improvement project to improve the integration of palliative care across sectors.
