Medical Wards
The GIM Faculty all work on the Medical Clinical Teaching Units (CTUs) providing acute care in the Emergency room and on the inpatient wards and D4ICU ‘step-down’ unit at Kingston Health Sciences Centre (KHSC). This is the single largest admitting service in the Hospital, divided up among 6 medical teams. Each team consists of an Attending physician, post-graduate medical trainees (residents), and medical students.
GIM Consult Service
The GIM Division also provides a 24/7 consultative service to the non-medical admitted patients at KHSC, (e.g. Psychiatry, Surgery, Obstetrics and Gynacology), for assistance with medical issues pertaining to their patients.
The Divison of General Internal Medicine faculty participate in the following outpatient clinics:
Urgent Clinic (Tue and Fri pm)
The GIM Division runs urgent clinics twice a week that patients who have acute medical issues that need addressing or following up can be referred. The patients will be reviewed by GIM Fellows and Staff.
General Internal Medicine (GIM) Clinic (Mon am, Thu am, Thu pm, Fri am)
The Division is responsible for several GIM clinics each week for post-hospital discharge care, new referrals and management of on-going multisystem medical problems. The patients are seen by Internal Medicine residents and Staff
Perioperative Clinics
The Division of General Internal Medicine runs two half day clinics per week dedicated to the care of pre-operative patients with internal medicine problems. The service sees patients for risk assessment and optimization prior to surgery. Clinics are run at the HDH PSS, in parallel to anesthesia, to facilitate discussion regarding complex cases. In addition, high-risk patients are identified for inpatient post-operative follow-up by the GIM consult service. Senior trainees in GIM participate in the clinics as part of their Royal College requirements.
Bariatric Clinics
Patients are seen both pre and post-operatively for optimization and medical issues related to surgery.
Obstetric Medicine Clinic (Fri am)
Faculty with an interest and additional training in medical problems in Pregnancy (pre and post-natal care) are followed in this clinic working collaboratively with the Ob/Gyn team.
- The GIM group at Queens has expertise in a broad range of diseases, and in particular specializes in the diagnosis and management of complex diseases.
- The GIM group currently runs a variety of clinics to which you can refer specifically, or your patient will be triaged to these clinics:
o GIM Resident Clinic: for general internal medicine concerns
o Fellow’s Urgent Clinic : run by PGY4 & 5s in GIM, supervised by an attending physician. For urgent assessments (<1month)
§ Procedures such as thoracocenteses and paracenteses can be performed in these clinics
o Perioperative Clinics: addressing stability and management of chronic medical conditions prior to surgery
o Obstetric medicine: managing medical conditions (eg Hypertension) in pregnancy (pre/ante/post partum)
o Bariatric Clinic: assessing and managing patients pre and post bariatric surgery with a variety of issues (must be a patient of the bariatric centre)
Specific issues that we can help with:
- Post-discharge follow up for patients who have recently been admitted to the Clinical Teaching Units (CTUs)
- Undifferentiated symptoms (eg SOBOE)
- Poorly controlled chronic diseases (eg diabetes, hypertension, COPD, liver disease)
- Abnormal bloodwork of unclear significance
- De-prescribing, medication reviews
- Refractory IDA requiring IV iron
- Please note:
o With regards to the question of ‘Chronic Lyme Disease’, the GIM clinic does not prescribe antibiotics for this condition and only endorses Lyme serology testing done by Public Health Ontario
o The GIM clinic generally does not prescribe narcotics, these are left to the discretion of the PCP
o GIM cannot provide primary care to those without a PCP

- To be seen by a GIM specialist, you require a referral from your primary care provider (PCP), MD or NP, or from another medical specialist
- Your referral will be reviewed by one of the consulting internists who will ‘triage’ the referral based on urgency and determine when you will be offered an appointment
- Depending on the urgency of your case, you will be generally offered an appointment between 1-4months of the initial referral date
What to expect at your appointment:
- Please bring all of your medications with you or an up to date medication list to your appointment
- You will first be seen by a nurse who will review your medications and who will take your weight, blood pressure and heart rate
- You will then be seen by an Internal Medicine Senior Resident (a trainee) who will review your medical history, your current problem and perform a physical exam
- The resident will review the case with the supervising or Attending physician who will then meet with you to discuss your diagnosis and the plan
- You may be asked to do further bloodwork or x-rays etc.
- You should expect to be in the clinic for ~60-90min.
- Depending on your problem, you may need further follow up in which case you will be contacted by the GIM office to book the appointment
- A letter documenting the encounter will be sent to your PCP within a few weeks of the clinic appointment.
