Palliative Care Community Consult Team
Assisting primary care teams with symptoms and care planning. We follow patients until end-of-life if challenging symptoms persist, providing 24/7 on-call support for those we follow.

Hospice Care at A.B. Smith Homestead House
A 9-bed unit providing holistic, compassionate hospice care for patients with a life expectancy < 1 month in a home-like environment with a focus on comfort and family support at the end of life.

Palliative Care Unit (PCU) at PCH
A 10-bed unit providing holistic, compassionate end-of-life care for patients with a life expectancy < 3 months. The focus is on symptom management, spiritual care, and quality of life.

Supportive and Palliative Care Clinic - Cancer Centre of Southeastern Ontario and Belleville Cancer Centre
Clinic supporting patients with cancer or cancer-treatment associated symptoms management, supports, and advance care planning.

KHSC Inpatient Consult Team
Direct support for patients admitted to KHSC. Our team provides consultative expertise for complex symptoms and assists with transitions to outpatient programs.
